How to Request Records
We understand that change can be difficult. If you wish to continue your care elsewhere, your new provider will likely ask for your medical record. Elevate Mental Health will serve as the custodian of your medical record. You can request a copy of your record or transfer your chart to another provider by completing a Release of Information form and returning via:
