About Elevate Mental Health
In 2025, Northlight Counseling Associates of Arizona, located in Phoenix and Mesa, and Phoenix Mental Health and Wellness of Arizona, located in Phoenix, Show Low and Flagstaff joined forces to become Elevate Mental Health. Together, Elevate Mental Health proudly provides comprehensive medication management and counseling services, both virtually and in-office, to individuals and families throughout Arizona.

Insurances Accepted
We are in-network with BCBS, Aetna, Tricare, Multi Plan, Optum, AHCCCS (AZ Complete Care, Pathfinder, & Care1st). Please reach out to our office or your plan carrier to confirm whether your insurance is accepted.
Cancellation Policy
Appointments must be cancelled at least two (2) business days prior to the scheduled appointment time. Clients can cancel their appointment by calling the main phone number to the center for psychiatric providers prior to the two (2) business day cutoff. There is a $100 charge for no-show and late cancellation of therapy and psychiatry appointments. There is a $300 charge for no-show and late cancellation of testing services. If you have a card on file, payment will be collected by the appropriate staff member. If you do not have a card on file, you will receive a bill.
